NEIGHBORING BUSINESS FOOD VENDOR APPLICATION

20th ANNUAL LOS FELIZ VILLAGE STREET FAIR

SATURDAY SEPTEMBER 7, 2013 12:00 PM TO 11:00 PM ON HILLHURST AVE. IN LOS FELIZ

Space will be assigned by the Street Fair Committee on a first come, first served basis, with an emphasis on attendee foot traffic flowing throughout the fairground.

Your exhibit package includes one (1) 10’ x10’ tent, including full back wall, 2 pony walls, one 8 foot table, 2 Samsonite chairs and one (1) 20 amp circuit/200 watts. FOOD VENDORS ALSO GET: a large, multi-compartment hot & cold water utensil-washing sink.

The event promoter has additional items available for rent, options and and prices are in the form below.

The BID and Street Fair Committee reserves the right to refuse participation to vendor of unsuitable merchandise and to limit the number of vendor/participants, or amend the floor plan at its sole discretion. Changing assigned locations onsite will not be permitted.

Please note: The Street Fair SOLD OUT last year so please do not wait until the last minute if you would like to participate.

INSURANCE: PLEASE PROVIDE A COPY OF YOUR CERTIFICATE OF INSURANCE

FOOD VENDOR REFUNDS: REFUNDS AT 100% WILL BE ISSUED IF CANCELLED BEFORE 12 NOON PST ON JUNE 30, 2013. THERE WILL BE NO REFUNDS AFTER JUNE 30, 2013 AT 12 NOON PST. ANY FEES PAID DIRECTLY TO THE LA COUNTY DEPT OF PUBLIC HEALTH MUST BE REFUNDED BY THAT OFFICE, ONLY FEES PAID TO THE LFVBID THROUGH PAYPAL OR WITH CASHIER'S CHECK OR MONEY ORDER CAN BE REFUNDED BY THE BID.

ATTN FOOD VENDORS: 1. YOU MUST PROVIDE YOUR OWN DEPT OF PUBLIC HEALTH APPROVED HAND WASHING SET-UP WITHIN YOUR BOOTH. 2. WE STRIVE TO NOT HAVE CONFLICTS BETWEEN LOS FELIZ VILLAGE RESTAURANTS AND GUEST FOOD VENDORS, AS IT WAS THE RESTAURANTS THAT STARTED THE FINE TRADITION OF THE STREET FAIR OVER 20 YEARS AGO. PLEASE BE AS DETAILED AS POSSIBLE WHEN LISTING WHAT YOU WILL BE SERVING AND SELLING, AS IT IS IMPORTANT TO HAVE A VARIETY OF FOODS AVAILABLE. FOOD VENDORS ARE RESPONSIBLE FOR THEIR OWN SIGNAGE, SET-UP AND DECORATION OF THEIR ASSIGNED AREA. 3. ONLY PERSONS UNDER YOUR EMPLOY SHOULD BE IN YOUR BOOTH AND NO SMALL CHILDREN FOR SAFETY REASONS. 3. WE RESERVE THE RIGHT TO REFUND 100% OF YOUR APPLICATION FEE, SHOULD WE FIND THAT THERE ARE FOOD VARIETY CONFLICTS WITH LOS FELIZ VILLAGE RESTAURANTS THAT CANNOT BE RESOLVED. WE MAKE EVERY ATTEMPT TO ENSURE THERE ARE A VARIETY OF FOODS PRESENT FOR SALE. FOR INSTANCE MORE THAN ONE VENDOR MAY SELL MEXICAN FOOD, IF THEY EACH SELL A DIFFERENT TYPE OF SPECIALTY.
